用免疫印迹技术研究儿童抗幽门螺杆菌循环抗体

Humoral immune response to Helicobacter pylori in children as determined by western blot technique

用活检粘膜细菌培养、快速尿素酶试验和病理切片HE染色等方法对60例有症状儿童进行了幽门螺杆菌（HP）感染状况的检测，在此基础上建立了检测抗HPIgG抗体的免疫印迹方法。引入半定量积分法表示结果，IgG积分≥2分为阳性，则免疫印迹方法检测抗HgIgG抗体的敏感性为91．1％，特异性为80．0％。用免疫印迹技术对儿童血清中抗HgIgG、IgA、IgM抗体进行分析并与成人相比较，结果发现：与成人相比，儿童IgG识别条带与成人相似，IgA识别条带少而浅，IgM识别条带多而深，在活检粘膜HP为阳性、免疫印迹IgG为阴性的4个病例中，3例lgM为阳性；在HP阳性患者中，儿童抗HPIPM抗体的阳性率（88·9％）远高于成人16·3％（P＜0·001），说明抗HPIPM抗体可指示HP早期感染。

60 children with dyspepsia were examined for Helicobacter pylori using culture, urease test and histologytechnique. Based on these results, western blot methods to detect IgG antibody to H. pylori was set up. The sensitivityand specificity of this method were gi. 1 % and 80. 0%, respectively. The recognition bands of IgG in children were138. 7kD, 100. 31kD, 91. 31kD, 85. 76kD, 71. 79kD, 67. 43kD, 64. 67kD, 57. 04kD and 48. 77kD. IgA bands were138. 71 kD and 100. 31kD. IgM bands were 85. 76kD, 71. 79kD, 64. 67kD, 57. 04kD and48. 77kD. The sera IgG recognition bands in children were similar to those in adults the number of recognition bands of IgA were fewer and the colorwas lighter than those in adults; whereas the number of IgM bands were more and the color was darker than those inadults. In some children with H. pylori infection, serum anti-H. pylori IgG antibody was negative but IgM was positivewith western blot technique. The positive rate of serum anti-H. pylori IgM antibody was much higher in children withH. pylori infection (88. 9% ) than in adults with H. pylori infection (16. 3%,P<0. 001). The presence of serum antiH. pylori IgM antibody suggests early stage of H. pylori infection. It is suggested that serum anti-H. pylori IgG andlgM antibodies should be used for detection of H. pylori infection in children.
