血清及卵泡液瘦素水平与IVF-ET中卵巢反应性的关系 被引量:1

摘要 目的观察血清与卵泡液瘦素(leptin)水平与体外受精-胚胎移植(IVF-ET)中卵巢反应性的关系。方法行IVF-ET患者30周期,在进入周期前早卵泡期检测血清瘦素水平,控制性超排卵后绒毛膜促性腺激素(hCG)注射日检测血清瘦素水平,经阴道取卵时留取卵泡液检测瘦素水平,同时观察患者早卵泡期促卵泡生成素(FSH)值、基础窦卵泡数、促性腺激素(Gn)用量,hCG注射日血清雌二醇(E2)水平、直径大于14mm卵泡数、直径小于14mm卵泡数,获得成熟卵母细胞数,分析其相关性。结果进入周期前患者血清瘦素水平平均为(15.12±2.23)μg/L,与早卵泡期血清FSH水平呈正相关,与卵巢内基础窦状卵泡数无明显相关性。hCG注射日血清瘦素水平为(17.32±3.14)μg/L,与卵泡液中瘦素水平相似(16.79±2.87)μg/L,二者均与hCG注射日血清E2值呈负相关,与Gn用量、直径大于14mm卵泡数呈正相关,直径小于14mm卵泡数无明显相关性,并与获得成熟卵母细胞数呈负相关。结论瘦素水平与卵巢储备窦泡数无明显相关性,但瘦素水平高,Gn用量多,成熟卵母细胞少,卵巢反应性降低。提示瘦素可作为卵巢对促排卵反应性的指标之一。
