潍坊地区妇女骨密度定量CT观察研究 被引量:2

摘要 目的通过观察潍坊地区健康妇女腰椎骨小梁的骨密度值,建立正常骨密度定量CT参考值,并探讨骨密度的定量cT的应用价值,为骨质疏松的预防、干预及骨折危险性的评价提供参考。方法选择在医院体检的潍坊地区健康妇女150例为研究对象,年龄20-69岁。将研究对象分成5个年龄组,每组30例。利用定量CT软件测量研究对象L1,L2,L3椎体小梁的骨密度,确定潍坊地区健康妇女骨密度参考值。结果潍坊地区-29岁的健康受试妇女L1-L3平均骨密度值为(168,86±17.34)mg/cm^3,~39岁组妇女骨密度值为(160.36±18.59)mg/cm^3,~49年龄段妇女的骨密度值为(143.35±19.56)mg/cm^3,50~59岁年龄段妇女的骨密度值为(120.31±18.36)mg/cm^3,60-69岁年龄段妇女骨密度值为(99.66±15.64)mg/cm^3,-59岁绝经前妇女的骨密度值明显高于绝经后妇女(P〈0.01)。骨质疏松的骨密度临界值为112.26mg/cm^3;骨密度年丢失百分率为1.58%;平均年丢失量为2.40mg/cm^3。结论使用定量CT可测量并建立该地区健康妇女的骨密度值,明确骨质疏松的临界骨密度值,为骨质疏松症的预防、干预、预后和骨折危险性的评价提供参考。 Objective To establish a normal BMD standard by observing lumbar BMD with quantitative computerized tomography (QCT) measurement in Weifang healthy women, so as to provide basis for clinical prevention and treatment of osteoporosis,and for assess of fracture risk in the region. To discuss the value of quantitative CT in measuring bone mineral density. Methods A total of 150 women from 20 to 69 years old, who received physical examination in the hospital were enrolled in this study. They were divided into five groups. Trabecular BMD of lumber bodies ( Ll - L3 ) were measured with QCT software so as to establish a standard of normal BMD in Weifang healthy women. Results The mean lumbar spine( L1 - L3 ) BMD values of five age groups from 20 to 69 were ( 168.86 ± 17.34), ( 160.36 ± 18. 59), ( 143.35 ± 19.56), ( 120.31 ± 18.36), (99.66 ± 15.64) ( mg/cm^3 ) respectively. The average lmnbar spine BMD value from 40 to 59 years ( 130.26 ± 20.35mg/cm^3 ) was greater( P 〈 0.01 ) in premenopausal women than in postmenopausal women( 100.51 ± 17.96mg/cm^3 ). Fhe threshold bone mineral density value for osteoporosis was 112.26mg/cm^3 ;The percentage of annual bone mineral density loss was 1. 58% ;The BMD loss per year was 2.40mg/cm^3. Conclusion We established a standard of normal BMC in women with QCT measurement, which provides the threshold bone mineral density value for osteoporosis, and provides basis for prevention and treatment of osteoporosis as well as evaluation of prognosis and fracture risk.
