脑胶质瘤颅外转移复发一例报告并文献复习

Extracranial metastasis and recurrence of intracranial glioma:a case report and review of literatures

目的探讨脑胶质瘤颅外转移复发的机制、临床特点及治疗方法。方法对1例肿瘤切除后颅外转移复发的脑胶质瘤病例分析其临床特征、影像学特点、手术所见、病理学结果及治疗方法。结果右颞顶少突胶质细胞瘤切除术25年后切口附近皮下异位复发,切除术后9个月后再次复发,并向腮腺、头颈部多发淋巴结转移。经手术、放疗和化疗等综合治疗,肿瘤部分缓解。结论脑胶质瘤可发生颅外转移复发,涉及机制复杂,应尽量手术治疗,并辅以放疗。

Objective To investigate the mechanisms, clinical characteristics and therapeutic strategies for extracranial metastasis and recurrence of intracranial glioma. Methods The clinical features, radiological characteristics （MRI/PET-CT）, surgical findings, pathological results and therapeutic methods in one case with extracranial metastasis and recurrence of intracranial glioma after tumor resection were retrospectively analyzed. Results The right parieto-temporal oligodendroglioma metastasized and recurred subcutaneously 25 years later near the incision of resection; 9 months later after later resection, the glioma recurred again and metastasized to parotid and multiple lymph nodes. With the combined therapy of surgery, radiotherapy and chemotherapy etc, symptoms were partially alleviated. Conclusion Intracranial glioma may metastasize and recur extracranially. Due to the complex mechanisms behind, surgical treatment should be executed with the assistance of radiotherapy and chemotherapy.
