启东肝癌高发区1972～1991年全人群肝癌生存率分析

Population based Survival Analysis of Primary Liver Cancer in a High incidence Area Qidong, China during 1972-1991

为获取启东全人群肝病的生存资料，对启东肝癌高发区１９７２～１９９１年以人群为基础的１１２２０例肝癌生存率及相对生存率作出分析。结果显示，第一个１０年（１９７２～１９８１年）的５年观察生存率为２．１％，第二个１０年（１９８２～１９９１年）为２．０％。而两个阶段的５年相对生存率分别为２．２％及２．３％。女性的生存率优于男性，第一阶段的男、女性５年生存率分别１．７％比３．５％，差异有显著性。第二阶段分别为１．８％比２．６％，差异无显著性。１９８２～１９９１年的资料还显示出年龄组的生存率差异无显著性，男性０～３４、３５～４４、４５～５４、５５～６４和６５岁以上组的５年生存率分别为２．２％、１．９％、１．３％、２．２％和１．５％，女性为３．７％、３．１％、２．８％、２．１％和２．６％。同样，男女性５年相对生存率分别为２．２％、１．９％、１．４％、２．４％、１．９％，及３．７％、３．１％、２．９％、２．２％、３．０％。表明全人群肝癌的预后甚差，亟待加强肝癌高危人群的筛检，早期发现及采取治疗措施，以提高肝癌的总体生存率。

Population based survival and relative survival of primary liver cancer (PLC) were analyzed for 11 200 cases in an area with high incidence, Qidong, Jiangsu Province, during 1972 to 1991. Results showed that observed survival was 2.1% and 2.0% in the first (1972-1981) and the second ten years (1982-1991), respectively, and relative survival 2.2% and 2.3%, respectively. Females had more favorable survival than did males, with five year survival of 3.5% and 1.7% in the first ten years, respectively, and 2.6% and 1.8% in the second ten years, respectively, but without statistical significance in their difference. No significant difference in age groups was found during 1982 to 1991, with five year survival of 2.2%, 1.9%, 1.3%, 2.2% and 1.5% for males, and 3.7%, 3.1%, 2.8%, 2.1% and 2.6% for females, and with five year relative survival of 2.2%, 1.95%, 1.4%, 2.4% and 1.9% for males, and 3.7%, 3.1%, 2.9%, 2.2%, and 3.0% for famales, in 0 34, 35 44, 45 54, 55 64 and 65 over age groups, respectively. It indicated that prognosis of PLC was poor in Qidong, and it is urgent to screen for PLC in high risk population, to detect it earlier, to manage it with adequate measures, and to improve its survival.
