卒中单元管理脑梗死患者近期疗效观察

Early curative effectiveness of cerebral infarction patients under the management of stroke unit

目的研究卒中单元管理脑梗死患者的早期疗效。方法289例住院的脑梗死患者随机进入卒中单元和普通病房,观察指标是病死率、住院时间、神经功能评价(NIHSS)、生活能力评价(BI)、社会功能评价(OHS),出院时患者或亲属填写医疗满意度。结合住院花费分析卒中单元的效果。结果卒中单元组和普通病房组出入院NIHSS平均差值、BI平均差值、OHS平均差值均有极显著性(P<0.01);且卒中单元组住院病死率低(P<0.05),住院满意度高(P<0.01)。结论对脑梗死患者的治疗,卒中单元管理模式明显优于传统的住院治疗模式。

Objective To investigate the early curative effectiveness of stroke unit （SU） on the patients with cerebral infarction. Methods 289 inpatients with cerebral infarction were randomly divided into SU group and general ward （GW） group. The key indexes recorded included mortality, hospital stay, National Institutes of Health Stroke Scale （NIHSS）, Barthel Index （BI）, Oxford Handicap Scale （OHS）. All the patients or their relatives at the time of discharge were asked to have the Visual Analogue Scales （VAS） which reflects patient satisfaction. All these data, in combination with cost and effect were analyzed with SPSS soft package. Results There was significant difference in mean gap scores between the day of admission and discharge （P〈0. 01） in NIHSS, BI and OHS. Mortality was lower （P〈0. 05）, and patients＇ satisfaction higher in SU group than in GW group （P〈0. 01）. Conclusion In comparison with traditional model, the patients with cerebral infarction can get more benefits from SU.
