成人幕上室管膜瘤X刀治疗效果分析

The efficacy analysis of linear accelerator-based stereotactic radiosurgery for adult supratentorial ependymomas

目的探讨X刀治疗颅内室管膜瘤的疗效。方法X刀治疗12例手术残余室管膜瘤患者,其中WHO分级Ⅲ级4例,Ⅱ级6例,Ⅰ级2例。3例在X刀治疗前或后接受了20 Gy分割外照射治疗。X刀中心剂量17.8～28 Gy,边缘剂量9～16 Gy。结果平均随访28个月。1、2、3年的无瘤进展生存率分别为91.7%、33.3%、25.0%。半数无瘤进展生存期为18个月。结论X刀对颅内术后残留室管膜瘤是一个有效的治疗措施。

Objective To investigate the therapeutic efficacies of patients with supratentorial ependymomas treated with linear accelerator-based stereotactic radiosurgery （SRS）. Methods Twelve patients with postoperative residual ependymomas were treated with linear accelerator-based SRS. Four patients had WHO grade Ⅲ tumors, 6 had grade Ⅱ tumors and another 2 had grade Ⅰ tumors. Three of the 12 patients received fractionation and external beam irradiation before and after X-knife radiotherapy, a median dose of 20 Gy. The dose for the X-knife treatment center ranged from 17.8-28 Gy and the peripheral dose was between 9-16 Gy. Results The median follow-up time was 28 months. The one-, two- and three- year relapse-free survival rates were respectively 91.7%, 33.3% and 25.0%. The median relapse-free survival time was 18 months. Conclusion Linear accelerator-based SRS is effective for the treatment of adults with postoperative residual supratentorial ependymomas.
