P53、P16基因在骨肉瘤中表达及临床相关性研究 被引量:8

摘要 目的探讨P53、P16抑癌基因在骨肿瘤中的表达及临床相关性研究。方法采用免疫组化染色及原位杂交方法检测105例骨肿瘤P53、P16蛋白及基因的表达,其中骨肉瘤85例、骨软骨瘤20例,并对骨肉瘤主要临床资料和病理分级参数进行比较,用x2检验进行统计学处理。结果骨肉瘤中P53蛋白及基因的阳性表达率为64.7%、65.9%,明显高于骨软骨瘤阳性表达率20.0%、20.0%,有显著性差异(P<0.0005)呈正相关。骨肉瘤中P16蛋白及基因的阳性表达率为52.9%、54.1%,明显低于骨软骨瘤阳性表达率85.0%、90.0%,两者之间有显著性差异(P<0.005,0.01),呈负相关。P53蛋白及基因在骨肉瘤IIb期、III期的表达率明显高于I、IIa期(P<0.0005)。病理分级P53蛋白及基因阳性表达率分别为I级35.0%、40%;II级64.7%、64.7%;III级83.8%、83.8%,有显著性差异(P<0.005)。软组织浸润和转移P53蛋白及基因阳性表达率明显高于非浸润和非转移组(P<0.0001,0.01)。P16蛋白及基因在骨肉瘤I、IIa期的表达率明显高于IIb期、III期(P<0.0005,0.001)。病理分级P16蛋白及基因阳性表达率分别为I级85.0%、85.0%;II级52.9%、55.9%;III级32.3%、32.3%,有显著性差异(P<0.001)。软组织浸润和转移P16蛋白及基因阳性表达率明显低于非浸润和非转移组(P<0.0005,0.005)。结论P53蛋白及基因在骨肉瘤中不同程度异常表达与骨肉瘤的病理分级、临床分期和浸润、转移密切相关,P16蛋白及基因在骨肉瘤中的异常表达与骨肉瘤的发生、发展中起重要作用,提示两种基因均可作为判断骨肉瘤进展及患者预后参考指标。 Objective To discuss the expressions of P53 and P16 in osteosarcoma and their significance. Methods Immunohistochemical technic and hybridization in situ were used for detection the expression of protein and gene of P53 and P16 in 85 osteosarcoma cases and 15 osteochondroma cases respectively. The results were compared with the main clinical pathological parameters in osteosarcoma. The significance of the expression was analyzed statistically. Results In osteosarcoma cases, the positive rates of protein and gene of P53 were 64.7% and 65.9% respectively, both positive rates were obviously higher than in osteochondroma cases of 20.0%, 20.0% respectively (P〈0.0005);and the positive rates of protein and gene of P16 were 52.9%, 54.1% respectively , both positive rates were obviously lower than in osteochondromacases of 85.0%, 90.0% respectively (P〈0.0005, P〈0.01). The expression of p53 were higher in clinical stage lib, Ⅲ than in stage Ⅰ,Ⅱa(P〈0.0005). And in difference pathological grade, expression of P53 proteins andgene were 35.0%, 40.0% in grade Ⅰ, 64.7%, 64.7% in gradeⅡ, 83.8%, 83.8% in grade Ⅲ respectively, the difference were significant(P〈0.005). The expressions of P53 was higher in cases with soft tissue invasion and metastasis than cases without those(P〈0.0001, P〈0.01). The expression of p16 were higher in clinical stage Ⅰ, Ⅱa than in stage Ⅱb, Ⅲ (P〈0.0005, P〈0.01). And in difference pathological grade, expression of P53 proteins and gene were 85.0%, 85.0% in grade Ⅰ, 52.9%, 55.9% in grade Ⅱ, 32.3%, 32.3% in grade Ⅲ respectively, the difference were significant(P〈0.001). The expressions of P16 was lower in cases with soft tissue invasion and metastasis than cases without those(P〈0.0005, P〈0.005). Conclusions P53 expression in osteosarcoma was positively correlated with clinical stage, pathological grade and invasion and metastasis, and P 16 expression in osteosarcoma play an important role in the occurrence and development. And the high expression of P53 and P16 perhaps can be used as markers to predict biological behavior of osteosarcoma and the prognosis of these patients as well.
