肺表面活性物质治疗早产儿肺透明膜病32例临床分析 被引量:2

The effect of curosurf on the 32 preterm infants suffered from hyaline membrane disease

摘要 目的探讨肺表面活性物质(固尔苏)治疗肺透明膜病的临床疗效。方法32例早产儿于出生后4 h内气管内灌注固尔苏,观察固尔苏治疗肺透明膜病的疗效。结果用药后30 min、6 h、12 h、18 h和24 h的血气分析数值与用药前比较,动脉血氧分压(PaO_2)、动脉血二氧化碳分压(PaCO_2)、动脉与肺泡氧分压比率(a/APO_2)、氧饱和度(SaO_2)、氧分压与吸人氧浓度比率(PaO_2/FiO_2)显著升高(P<0.01)。用药后6 h、12 h的呼吸机调节参数逐渐降低,与用药前比较,差异有统计学意义(P<0.01)。用药后6 h、24 h经胸部X线摄片检查,无一例早产儿出现肺透明膜病征象。结论出生后尽早使用固尔苏,可预防、治疗肺透明膜病,改善肺氧合功能。 Objective To study the effect of curosurf on the hyaline membrane disease. Methods The 100mg/kg surfactant was dripped into the trachea in less than 4 hours after birth and the therapeutic effect was examined on the hyaline membrane disease. Results Blood gas analysis was taken at half an hour,six,twelve, eighteen and twenty-four hours after the surfactant, respectively. The values of PaO2, SaO2 and PaO2/FiO2 were strikingly increased, which were all significantly improved than those of pre-surfactant(P 〈 0.01 ), and the parameters of ventilator were gradually decreased at six and twelve hours post-surfactant,the differences were both more remarkable than those of pre-surfactant(P 〈 0.01 ). At six and twenty-four hours after the surfactant the preterm infants were all examined with X-ray, and none of them developed hyaline membrane disease. Conclusion Curosurf used as soon as possible after birth can prevent hyaline membrane disease and improve the alveolar oxygenation.
