乳腺癌术后患肢功能锻炼效果的评价

Evaluation of post-surgical function of dirigation in wounded limbs of patients with breast cancer

目的探讨乳腺癌术后由护士指导、参与的个体化护理对患肢功能恢复以及预防术后并发症,促进患者早日康复的作用。方法选择该院1998年1月至2006年12月乳腺癌手术患者121例。将其随机分为训练组(61例)和对照组(60例)。两组均在术前1～3 d进行术前宣教。手术后,对照组给予常规护理,训练组进行个体化护理及患者主动性的功能锻炼。结果在第一阶段(术后第1～10天)的功能锻炼中,训练组全部达到预期目标,对照组仅有54例达到预期目标。在第二阶段(术后第11～14天)的功能锻炼中,训练组有60例达到预期目标,对照组仅有52例达到预期目标。训练组中只有1例出现手术并发症,对照组10例发生并发症。两组间差异均有统计学意义(P<0.05)。结论护士实施的个体化护理及患者主动性的功能锻炼,可促进乳腺癌术后患者患肢运动功能的恢复,减少术后并发症,提高乳腺癌患者术后的生活质量。

Objective To probe into the clinical function of individualization nursing in which nurses offer introduction and participation to enhance the recovering function of wounded limbs, and to prevent post-surgical syndrome and promote earlier recovery of patients. Methods 121 cases of breast cancer surgical operation from January 1998 to December 2006 were chosen in this hospital. All the chosen cases were randomly divided into two groups, i. e. the experimental group（61 cases） and the control group（60 cases）. Two groups were instructed 1 or 3 days before surgery. After surgery, patients in the control group were treated with normal nursing, while experimental group with individualization nursing and patient＇s active dirigation. Results The research showed that in the first term of dirigation（from 1st day to 10th day after surgery） all 61 cases of the experimental group achieved the expected result, while only 54 cases of the control group got the same result. The research also showed that in the second term（from 1 lth day to 14th day after surgery） 60 cases of the experimental group got expected result in their dirigation while only 52 cases of the control group did so. Only 1 case of the experimental group had the complication disease while 10 cases of the control group did so. The difference between two groups had statistical significance（P〈 0.05）. Conclusion Individualization nursing by nurses and active dirigation by patients could greatly improve the movement function of wounded limbs of patients after breast cancer surgical operations, promote their restoring function, decrease complication diseases,ease patients＇ suffering,and help promote patients＇ quality of life.
