芬太尼复合氯诺昔康用于妇科术后静脉镇痛 被引量:4

摘要 目的评价芬太尼复合氯诺昔康用于妇科下腹部手术患者术后静脉镇痛的效应。方法40例全身麻醉下行妇科下腹部手术患者,以术后静脉镇痛用药不同,随机均分为两组。分别记录两组术后2、6、24、48h的视觉模拟评分(VAS),患者对镇痛效果的总体满意度及不良反应发生情况。所有患者分别于麻醉前、术毕、术后4、24和48h5个时点在非输液侧抽取静脉血用双抗体夹心桥联法(ABC-ELISA)测定皮质醇(Cor)、催乳素(PRL)、白细胞介素-6(IL-6)和IL-10。结果两组术后各时点VAS差异无统计学意义。镇痛效果的总体满意度A组高于B组,但两组之间差异无统计学意义。B组恶心、呕吐的发生率显著高于A组(P<0.05或P<0.01)。A组Cor水平在术后4、24、48h明显低于B组(P<0.05)。A组术后IL-10水平高于B组,其中在术后4、48h与B组相比差异有统计学意义(P<0.05)。PRL术毕较术前明显升高(P<0.01),但各组之间差异无统计学意义。结论芬太尼复合氯诺昔康用于妇科下腹部手术患者术后静脉镇痛效果可靠,与单纯芬太尼相比不良反应明显减少明显降低术后疼痛导致的应激反应。
