摘要
目的:通过分析血吸虫血清学室间质量评价,探讨影响实验室结果相关因素,提高检验人员的质控意识及免疫学实验室的水平和质量。方法:全年组织室间质评两次,一次发放5个血清质控样,要求各实验室按规定的时间检测并回报结果,一次现场抽取10%查病血样复查,计算符合率及Kappa值,采用鱼骨图法对血吸虫病血清流行病学调查质量控制进行评价。结果:2002~2006年武汉市血吸虫病外质控血清样和Kappa值分别为82.8%、87.8%、92.6%、93.5%、91.4%和0.61,0.73,0.82,0.86,0.82;复查血清样品符合率和Kappa值分别为86.1%、91.2%、96.7%、95.5%、93.9%和0.70,0.73,0.93,0.90,0.88。结论:全市的室间质控结果有一定的可比性,为全市血吸虫病实验室提供了技术和信息平台,对于提高实验室检测水平有一定帮助,血吸虫查病质量有了显著提高,能反映出血吸虫病检测水平。
Objective:To analyze the quality of serological test of schistosomisis among different laboratories,and to discuss related factors which affect the result of test and raise checkers' consciousness of quality control and improve the test quality of the immunological labs.Methods:Twice quality assessments were carried out among different labs per year.One time 5 serum samples of quality control were provided to different labs to examine,and results were required to report at a fixed time.The other time,10% serum samples were reexamined through field sampling.Coincidence rate and Kappa value were calculated.Fishbone chart method was used to study on control the quality of seroepidemiological survey of schistosomisis.Results:It was found that the coincidence rate of the serum samples of quality control were 82.8%,87.8%,92.6%,93.5%,and 91.4% respectively from 2002 to 2006,and Kappa value were 0.61,0.73,0.82,0.86 and 0.82,respectively.The coincidence rate of the reexamined serum samples were 86.1%,91.2%,96.7%,95.5%,and 93.9% respectively from 2002 to 2006,and Kappa value were 0.70,0.73,0.93,0.90 and 0.88,respectively.Conclusion:The results of quality control among different labs in the whole city can make a comparison,which offered the labs a platform of technology and information.And it was also helpful to improve the level of examination.The improved quality of survey can reflect the examination level.
