原发性高血压伴2型糖尿病患者血清同型半胱氨酸水平和胰岛素抵抗的关系 被引量:4

Correlation between serum homocysteine level and insulin resistance in patient with essential hypertension complicated with type 2 diabetic mellitus

摘要 目的:探讨原发性高血压伴2型糖尿病患者同型半胱氨酸(Hcy)水平与胰岛素抵抗(IR)的关系。方法:将64例原发性高血压患者分为伴2型糖尿病组38例,不伴2型糖尿病组26例,以健康志愿者38例为正常对照组,测定各组血清Hcy水平;并测定所有原发性高血压患者的空腹血糖、空腹血浆胰岛素浓度,计算胰岛素敏感性指数(ISI)。结果:伴2型糖尿病组患者的Hcy水平显著高于正常对照组和不伴2型糖尿病组(均P<0.01)。高Hcy血症患者的2型糖尿病的发生率高于无高Hcy血症者(P<0.01)。原发性高血压伴2型糖尿病患者血清Hcy与ISI呈显著负相关(r=-0.929,P<0.01)。结论:高Hcy水平可能为原发性高血压伴2型糖尿病的原因之一;IR是影响血清Hcy水平的危险因素。 Objective:To investigate the correlation between homocysteine(Hcy) and insulin resistance(IR) in patients with essential hypertension complicated with type 2 diabetic mellitus. Method: Sixty-four essential hypertension patients were divided into thirty-eight patients complicated with type 2 diabetic mellitus and twenty-six without type 2 diabetic mellitus. Serum Hcy levels and fasting serum insulinand fasting blood glucose was determined in all patients. Insulin sensitivity index(ISI) as detected. Result:The serum Hcy level[19.03±2.13] in essential hypertension patients complicated with type 2 diabetic mellitus was significantly higher than those[13.07±2.64] in patients without type 2 diabetic mellitus(P〈0.01). The incidence of type 2 diabetic mellitus in patients with high serum Hcy levels was higher than that in patients without high serum Hcy levels(P〈0.01 ). In essential hypertension patients complicated with type 2 diabetic mellitus negative correlation was found between the serum Hcy and ISI(r= -0. 929, P〈0.01). Conclusion.. It is possible that Hcy is one of the cause of essential hypertension complicated with type 2 diabetes mellitus. Insulin resistance is a dangerous factor influncing the serum homocysteine level.
