介入栓塞治疗四肢肌间血管瘤的远期疗效

Long-term effect of transarterial embolization for intramuscular hemangioma of extremities

目的探讨介入栓塞术在四肢肌间血管瘤治疗中的价值。方法在DSA引导下对24例四肢肌间血管瘤患者共施行31次超选择血管内栓塞治疗，其中上臂或肩部5例，前臂2例，大腿14例，小腿3例，全部病例随访26～74个月。结果所有病例的载瘤动脉及其周围畸形血管团得到全部或大部分闭塞，临床症状得到不同程度缓解，未出现严重并发症。22例肿块均有不同程度的缩小，2例无明显变化；其中13例稳定至今，5例于肿块缩小后外科手术完整切除，6例复发增大者经瘤内注射硬化治疗后稳定。结论介入栓塞治疗四肢肌间血管瘤安全有效，联合外科手术或瘤内注射等可取得更满意的疗效。

Objective To study the significance of interventional embolization on intramuscular hemangioma（IMH） in extremities. Methods Guided by DSA, superselected intraartery embolization（thirty-one times） were administrated to twenty-four patients with IMH in extremities. There were five cases with the lesion in the upper arm or shoulder, two in cubitus, fourteen in thigh and three in crus. All cases were followed up for from twenty-six to seventy-four months. Results Complete or subtotal embolization were obtained in both feeding artery and peripheral malformed vessels of the lesions. The clinical symptom of all patients were catabatic in different degree and no serious complications appeared. The tumor was dwindled in twenty-two cases in different degree and unchanged in two cases. In which, thirteen cases were found steady, five cases were carried out with surgical operation and the tumor was resected fully after it was dwindled resected fully, six cases were found recrudescent and then steady after carried out with percutaneous sclerosis therapy. Conclusion Interventional embolization for patients with IMH in extremities is safe and effect. More satisfying efficiency could be obtained by combining with surgical operation or percutaneous sclerosis therapy, and so on.
