C-erbB-2在子宫、卵巢特殊类型癌中的扩增

摘要 目的:研究子宫、卵巢特殊类型癌C-erbB-2的扩增以及与临床病理特征和患者预后的关系。方法:应用鉴别聚合酶链反应(DPCR)检测子宫、卵巢特殊型癌标本23例C-erbB-2癌基因的扩增。结果:23份中,16份(69.5%)含有扩增的C-erbB-2(2~8倍不等),其中子宫内膜透明细胞腺癌、子宫恶性苗勒管混合瘤及卵巢的子宫内膜样癌全部含有扩增的C-erbB-2;子宫乳头状浆液性癌、子宫内膜间质肉瘤、子宫内膜腺棘癌、腺鳞癌中则部分有扩增。组织学分化差、临床期别晚的患者有较高的C-erbB-2扩增率。术后随诊6~48个月,无瘤生存13例中,9例C-erbB-2扩增、占69.2%;带瘤生存及死于肿瘤的10例中,8例C-erbB-2扩增、占80%,但差异无显著性(P>0.05)。结论:同常见典型的子宫内膜腺癌一样,C-erbB-2在子宫、卵巢特殊型癌中亦有较高的扩增率。C-erbB-2扩增对预测此类肿瘤患者预后的价值,有待进一步研究。 Objective To determine the amplification of the C-erbB-2 oncogene in uncommon malignant tumors of uterus and ovary as well as its relationship to clinicopathologie features and prognosis. Methods Amplification of C-erhB-2 was studied by differential polymerase chain reaction (DPCR) in paraffin-embedded tumor tissues from 23 patients with uncommon tumors of uterus and ovary. Results 16 samples (69. 5%) were found to contain amplified C-erbB-2 (2-8 copies)by the DPCR method. All of samples with the clear cell carcinoma and malignant mullerian mixed tumor of the uterus, and the endometrioid tumors of the ovary were demonstrated the presence of the amplified C-erbB-2. The other types of carcinoma, such as serous adenocarcinoma,endometrial stroma sarcoma, and adenosquamous carcinoma had only partially amplified genes. It showed that less differentiated tumors with advanced stage were likely to have a high C-erbB-2 amplification. The follow-up was carried out in 6-48 months. The patients, alive with disease or dead of disease, had higher rate of amplification (100%, 80%)than those of patients, alive without disease (6 9.2%), but the difference was no statistical significance. Conclusion As in the common endometrial carcinoma, the C-erbB-2 is also frequently present in uncommon malignant tumors of the uterus and ovary. Patient's prognosis value needs further study.
