症状性椎管内硬膜外脂肪过多症的临床分析(附2例报道)

The clinical features of symptomatic spinal epidural lipomatosis

目的探讨症状性椎管内硬膜外脂肪过多症的相关临床问题。方法通过分析2例症状性椎管内硬膜外脂肪过多症患者的临床和手术后随访资料,结合文献探讨其病因以及诊断治疗方法。结果2例脊髓压迫症状严重的椎管内硬膜外脂肪过多症患者手术后神经功能得到改善。结论症状性椎管内硬膜外脂肪过多症可以是特发性或者激素诱导性,和肥胖有较为密切的关系,MRI是敏感特异的诊断方法,椎板减压和硬膜外脂肪切除手术能够改善部分患者的神经功能。

Objective To discuss the clinical features of symptomatic spinal epidural lipomatosis. Methods Two patients with symptomatic spinal epidural lipomatosis were analyzed its clinical features, postoperative follow up data, and some literatures were reviewed. The causes, methods of diagnosis and treatment were discussed. Results The neurological function of these two patients with severe spinal cord compression was improved postoperatively. Conclusions Spinal epidural lipomatosis might be idiopathic or induced by steroids, and is closed related with obesity, magnetic resonance image is sensitivie and specific diagnosis method, decompressive laminectomy with resection of epidural fat tissue can improve the neurological function of some patients.
