超声心动图与心电图判定左室心肌梗塞部位和范围对比研究

The Study of Comparing Two-dimensional Echocardiography with Electrocardiograph Used to Determine the Location and Range of Myocardial Infarction

目的:比较二维超声心动图(2DE)和心电图(ECG)对心肌梗塞(MI)定位诊断的异同、优劣。方法:对59例 MI 患者进行了2DE 和 ECG 检查。在2DE 上将左室分为16个节段而分别确定与 ECG 各导联的对应关系,分别计数2DE 上室壁运动异常(WMA)节段数和 ECG 上有异常 Q 波导联所对应节段数,对比分析两种方法检查结果的一致性。结果:2DE 和 ECG 分别检出338和311个节段阳性,两种方法阳性一致率57.9%,两种方法阳性一致率在各壁的高低依次为下壁、前壁、后壁、前间隔、侧壁;两种方法检测结果的差别有显著性(P<0.05);两种方法检测结果的相关系数 r=0.595(P=0.0000)。结论:两种方法的检查结果有较好的一致性,2DE 优于 ECG。

Objective:The aim of the study was to compare two-dimensional echocardiography (2DE)with electrocardiograph(ECG)used to determine the location and range of myocardial infarction. Methods:59 patients with MI were examined by 2DE and ECG.The left ventricnlar was divided into 16 segments on 2DE and the relation between each segment and responding lead on ECG was determined. Then the number of segments with wall motion abnormality(WMA)on 2DE and those which there were abnormal Q waves at responding leads on ECG calculated respectively.The accordance of the results from both methods was compared and analyzed.Results:The numbers of MI segments detected by 2DE and ECG were respectively 338 and 311.The concordant rate between the results of examination using 2DE and ECG was 57.9%.The order from high to low of the accordant rates between the results from 2DE and ECG in every wall was follw:inferior,anterior,posterior,anterior septum and lateral.The difference be- tween those was significant(P<0.05).The correlation coefficient of those r=0.595(P=0.000). Conclusion:There was a good accordance between the results of examination using 2DE and ECG;The positive rate of 2DE was higher than one of ECG.
