慢性阻塞性肺疾病中氧化应激的研究

摘要 目的:通过测定慢性阻塞性肺疾病(COPD)患者和正常对照组血清中8-差向前列腺素的水平,探讨COPD患者中氧化应激的水平。方法:20例急性加重期和缓解期的COPD患者、16例吸烟对照和24例不吸烟对照,COPD急性加重期和缓解稳定期、正常对照组分别采集外周静脉血离心,血清中游离8-差向前列腺素的测定采用酶分析法(EIA)。结果:COPD患者主要肺功能FEV1%和FEV1/FVC(分别为50.1±17.27和56.2±10.31)与对照组(分别为106.0±20.8和83.4±6.34)比较明显降低,结果差异有统计学意义(P<0.001)COPD急性加重期血清中游离8-差向前列腺素的浓度(5.42±2.416ng/ml)明显高于稳定期的浓度(3.30±2.267ng/ml),差异有统计学意义(P<0.01);吸烟对照组COPD稳定期血清中游离8-差向前列腺素的浓度(5.72±2.195ng/ml)高于COPD稳定期(3.30±2.267ng/ml)和不吸烟对照组(4.30±1.964ng/ml)的浓度,结果差异有统计学意义(P<0.05)。COPD血清中游离8-差向前列腺素和FEV1%、FEV1/FVC无显著相关性(r分别为-0.076和-0.046,P>0.05)。结论:在急性加重期的COPD患者和健康吸烟者,存在氧化应激水平的明显升高。COPD患者中氧化应激负荷可能和气流阻塞无相关。 Objective: To investigate the oxidative stress burden in patients with chronic obstructive pulmonary disease (COPD). Methods: Serum samples of 20 patients with COPD in severe exacerbation and stable stage, 16 healthy smokers and 24 healthy non - smokers were collected for measurements of the level of free 8 - isoprostane. The free 8 - isoprostane in serum was measured by enzyme immunoassay(EIA). Results: Significantly decreased FEV1% AND FEVI/FVC were shown in COPD patients,as compared to the healthy control group[ (50.1 ± 17.27)vs. (106.0± 20.82),P〈 0.01 and (56.2 ± 10.31)vs. (83.4 ± 6.34),P〈 0.01 ,respectively]. In severe exacerbation of COPD,the level of free 8 - isoprostane was significantly higher than stable stage of COPD(5.42 ± 2.416ng/ml, 3.30 ± 2. 267ng/ml, respectively, P 〈 0.01 ). In healthy smokers, the level of free 8 - isorostanein serum was higher than healthy non- smokers[ (5.72±2. 195)ng/ml, (4.3 ± 1.964) ng/ml, respectively, P〈 0.05]. The level of free 8- isoprostane in serum had nocorrelation with FEVI % and FEVI/FVC in patients with COPD( r = -0.076 and -0.046, respectively, P 〉 0.05 ). Conclusion: Increased systemic oxidative stress occurs in patients with severe exacerbation of COPD and in the healthy smokers. The oxidative stress burden may not correlate significantly to the obstruction of airways.
