急性呼吸窘迫综合征的肺复张方法与临床评价 被引量:7

摘要 随着对急性呼吸窘迫综合征(ARDS)病理生理学改变的认识以及机械通气治疗的不断进展,人们已经清楚地了解到,作为ARDS主要治疗手段的正压通气,可能导致的吸气末肺泡过度膨胀和呼气末肺泡萎陷,使肺损伤进一步加重,即产生呼吸机相关性肺损伤(VILI)。在此基础上提出了肺保护性机械通气策略,主要基于以下两个概念,一方面通过呼气末正压(PEEP)维持肺泡开放,防止肺泡的周期性开闭。另一方面降低潮气量,以避免吸气末过高的肺泡牵张压和肺泡过度膨胀。
