摘要
目的介绍应用微型可吸收骨锚治疗锤状指的临床疗效。方法对19例指伸肌腱终腱开放伤患指一期应用微型可吸收骨锚修复重建15例,二期修复4例;7例闭合性损伤,2周内修复6例,4周内修复1例。根据终腱止点远端残留的长度及伴肌腱缺损的情况采用4种缝合方式,术后外固定3~6周。结果术后26例获得3~11个月的时间随访,平均6.4个月。参照TAM系统评定法评定:优14例,良8例,可3例,差1例。3例于术后6~8周,1例于术后10个月出现远侧指间关节背侧皮肤红肿、疼痛等并发症。结论采用微型可吸收骨锚治疗锤状指疗效确切,操作简单,可以推广应用。
Objective To introduce the clinical outcomes of treating mallet finger using absorbable mini bone anchors (Mitek). Methods Nineteen cases of open injury of the terminal tendon of extensor apparatus were treated by tendon to bone fixation using absorbable mini bone anchor. Fifteen fingers had primary repair in the acute phase of debfidement while 4 fingers were repaired secondarily. Seven cases of dosed injury of the extensor tendon, 6 were repaired within 2 weeks of injury and 1 was repaired in 4 weeks. Four types of suturing methods were used based on the length of residual tendon insertion and the status of the tendon defect. All cases received external splinting for 3 to 6 weeks postoperatively, Results Twenty-six cases were followed-up for 3 to 11 months, with an average of 6.4 months. Evaluated with TAM system, the result was excellent in 14 cases, good in 8 cases, fair in 3 cases and poor in 1 case, Redness, swelling and tendemeas at the dorsum of the distal interphalangeal joint was noted in 3 cases 6- 8 weeks postoperatively and in 1 case 10 months after the surgery. Conclusion Absorbable mini bone anchor fixation is a reliable method to treat mallet finger, The operation is straightforward so it can be easily adapted.
