两种途径切除非脱垂子宫的对比分析

Clinical Analysis of Transvaginal and Transabdominal Hysterectomy for Non-prolapsed Uterus

目的对比非脱垂子宫经阴道与经腹切除术的临床效果,探讨非脱垂子宫经阴道切除术的优势及手术要点。方法选择2004年6月至2005年3月在解放军第153医院住院的非脱垂子宫疾病患者,行阴式全子宫切除术(TVH)42例,行腹式全子宫切除术(TAH)76例进行对照,对两组的手术时间、术中出血量、肛门排气时间、术后体温、伤口疼痛程度及住院时间等方面的差异进行研究。结果TVH组的手术时间与TAH组比较差异无显著意义(P>0.05);术中出血量、肛门排气时间、术后体温、伤口疼痛程度及住院时间均小于TAH组,两组差异有显著性(P<0.01)。结论TVH损伤小、恢复快,是一种较好的手术方法。

Objective To discuss the effect of transvaginal and transabdominal hysterectomy for non - prolapsed uterus. To assess the advantages, operative postulates of transvaginal for non - prolapsed uterus. Methods The transvaginal hysterectomy （TVH） for non -prolapsed uterus in 42 cases were compared with the transabdominal hysterectomy （TAH） in 76. They were chosen randomly from The 153 Hospital of Liberation Army from 2004 -6 to 2005 -3. The differences were studied, ineluding the average operation time, the operative bleeding, the intestines, function, temperature after operation, postoperative pain and hospitalization time. Results There was no significant difference in the operation time in the two groups （ P 〉 0.05 ） . The TVH group had a significant reduction in volume of blood loss, There was significant difference in the two groups （P 〈 0. 01 ） in the first time to break wind, temperature after operation, postoperative pain and hospitalization time. Conclusion With less damage and quicker recovery, the TVH is a good operative method.
