摘要 目的探讨血压水平和高血压类型与颈动脉血流速度的关系。方法选择脑卒中队列研究人群中55~65岁年龄段,无脑卒中、心脏病和糖尿病史者1084例,依据基线调查时的收缩压、舒张压水平和高血压类型进行分组,其中收缩压按每增加10mmHg(1mmHg=0.133kPa)分为9组,舒张压按每增加5mmHg分为8组,根据血压水平分为4种类型,比较和分析各组间颈动脉血流速度指标的差异。结朵当收缩压≥120mmHg或舒张压≥80mm Hg时,脑血管最大血流速度(Vmax)、最小血流速度(Vmin)和平均血流速度(Vmean)均开始呈逐渐下降趋势;当收缩压≥160mmHg或舒张压≥100mmHg时,脑血流速度下降的趋势更加明显。不同类型高血压组间的流速差异主要表现为左侧Vmin和右侧max,各组的流速均显著低于血压正常患者(P〈0.01)。结论从脑供血角度,理想的血压水平应〈120/80mmHg,当血压水平≥160/100mmHg时,颈动脉血流速度明显减慢。 Objective To probe into relationship between blood pressure level,type of hypertension and blood flow velocity of. carotid artery. Methods 1 084 participants aged 55 to 65 years and without history of stroke,heart disease and diabetes were selected from a Stroke cohort. The participants were grouped according to systolic and diastolic blood pressure levels and types of hypertension,i, e. ,the participants with various systolic pressures were divided into nine groups with 10 mm Hg difference. They were divided into eight groups according to 5 mmHg difference of diastolic pressure, and were divided into four groups according to types of hypertension. The blood flow velocity of carotid artery was compared between groups. Results The parameters, such as Vmax,Vmin, Vmean decreased gradually as systolic blood pressure 3 120 mm Hg or diastolic blood pressure 380 mm Hg. When systolic blood pressure was 3160 mm Hg or diastolic blood pressure was 3 100 mm Hg, the extent of decrease in the parameters was obvious. The parameters showing differences among different types of hypertension were Vmin of left carotid and Vmax of right carotid. Conclusions In view of carotid blood flow, the ideal blood pressure is 〈120/80 mm Hg. The velocity of carotid blood flow decreases significantly when the blood pressure is 3 160/100 mm Hg.
