前列腺汽化电切术患者麻醉前后安置体位的比较

Study on opportunity chosen of setting lithotomy position in patients undergoing transurethral electrovaporization of prostate

目的探讨前列腺汽化电切术患者硬膜外麻醉前安置膀胱截石位的效果。方法选取经尿道前列腺汽化电切术患者82例,随机分为观察组42例,对照组40例,观察组在硬膜外注入麻醉药物前安置膀胱截石位,对照组按常规方法硬膜外麻醉后安置膀胱截石位,比较两组患者术后腰部和下肢肌肉酸胀、麻木不适等情况。结果观察组33例术后无腰部和下肢肌肉酸胀、麻木不适主诉,顺利接受改进后安置的手术体位,术后不适有明显改善,两组比较有显著性差异(P<0.01)。结论硬膜外麻醉前安置手术体位能有效地减轻或预防腓总神经损伤。

Objective To explore the proper opportunity of setting lithotomy position in patients undergoing transurethral electrovaporization of prostate（TUVP）. Methods A total of 82 cases of patients undergoing TUVP were randomly divided into the observation group（42 cases） and the control group（40 cases）. Patients in the observation group were positioned in lithotomy position before epidural ansesthesia and those in the control group were positioned in litbotomy position after epidural ansesthesia. The occurrence of common peroneal nerve injury and patient comfort in the two groups were observed and compared. Results There were significant differences with regard to patients comfort and the occurrence of common peroneal nerve injury between the two groups（P〈0.01）. Conclusions Setting lithotomy position in patients undergoing TUVP before epidural ansesthesia can effectively reduce the occurrence of common peroneal nerve injury and improve the patient comfort.
