摘要
[目的]探讨影响产后抑郁发生的危险因素,为干预产后抑郁提供理论依据。[方法]采用围产期孕妇一般情况调查表、爱丁堡产后抑郁量表(EPDS)、生活事件量表(LES)、A型行为问卷(TABQ)、艾森克个性问卷(EPQ)、社会支持评定量表(SSRS)、症状自评量表(SCL-90)、特质焦虑量表(T-AI)、状态焦虑量表(S-AI)对129名围产期孕妇进行跟踪调查,直至分娩,并进行统计学分析。[结果]产后抑郁组受教育年限低于正常组;EPQ神经质原始分、SSRS客观支持分、怀有戒心或敌意特征、产妇年龄是影响产后抑郁发生的危险因素。高神经质组产前Sc l-90抑郁分、焦虑分、强迫分、躯体化等分值及负性生活事件分值均高于低神经质组,社会支持总分及利用度均低于低神经质组。[结论]神经质、低客观支持、怀有戒心或敌意及产妇年龄是产后抑郁发生的危险因素。
[ Objective] To explore the risk factors influencing the occurrence of post partum depression,to offer theoretical support for intervention of post partum depression. [ Methods] 129 gravidas were investigated by using the gravida general situation questionnaire, the Edinburgh Postnatal Depression Scale( EPDS), life event scale( LES) ,Type A Behavior pattern ques- tionnaire ( TABQ ) ,Eysenck Personality Questionnaire ( EPQ ) ,Social Support Rating Scale ( SSRS ), symptom checklist - 90 ( SCL - 90) ,Trait Anxiety Inventory( T -AI),State Anxiety Inventory( S -AI)in the earlier period of pregnancy till giving birth. The data were analyzed by statistics. [ Results ] The years of education received by the post partum depressin group was fewer than those of the control. The multiple regression analysis on the post partum depression displayed that the EPQ nervousness primitive score, SSRS objective support score, vigilance and hostility, and parturien age were risk factors that could influence the occurrence of the post partum depression. The score of antepartum SOl -90 depression,anxiety,compulsion somatization, and the score of negative life event were significantly higher in the group of high nervousness than in the control group. But the social support score and utilization degree was lower in the group of high nervousness than in the control group. [ Conclusion] Post partum depression was influenced by nervousness, objective support, vigilance and hostility and age at parturition.
