困难气管插管两种插管方法67例临床分析 被引量:1

摘要 目的:①探索方便、简单、可靠Cormack及LehaneⅢ、Ⅳ级困难气管插管的方法;②抑制气管插管所引起心血管反应,维持循环功能稳定。方法:选择Cor-mack及LehaneⅢ,Ⅳ级气管插管全身麻醉患者67例,年龄17~65岁,ASAⅠ、Ⅱ级,随机分为:TCI组,37例,丙泊酚、舒芬太尼血浆、效应室靶控诱导,铲"L"状气管导管插管;AA组,30例,丙泊酚、芬太尼人工诱导,鱼钩"C"状气管导管插管;观察:插管成功率对比,插管后心血管反应心率、平均动脉压。结果:①铲"L"状气管导管插管明显优于鱼钩"C"状气管导管插管方法,P<0.05;②TCI组有效抑制气管插管所引起心血管反应优于AA组,P<0.01。结论:铲状"L"气管导管插管方法方便、简单、可靠,适应范围广;丙泊酚、舒芬太尼靶控输注诱导有效抑制气管插管所引起心血管反应,循环功能稳定。
