大肠癌组织中SMAD4、CyclinD1、Survivin和Cox-2表达及意义

摘要 目的探讨SMAD4、CyclinD1、Survivin和Cox-2在大肠癌组织中的表达及意义。方法运用免疫组化SP法检测SMAD4、CyclinD1、Survivin和Cox-2在44例大肠癌组织中的表达,并建立多因素COX比例风险模型分析患者的预后。结果SMAD4、CyclinD1、Survivin和Cox-2表达与患者性别、年龄、大体类型和组织分级均无关(P〉0.05);SMAD4、Survivin及Cox-2表达与淋巴结转移和Dukes分期有关(P〈0.05),而CyclinD1表达与此二者无明显相关性(P〉0.05)。COX生存分析提示SMAD4表达与生存期呈正相关。结论联合检测SMAD4、CyclinD1、Survivin和Cox-2对了解大肠癌的发生、发展,指导治疗和判断预后有重要意义。SMAD4可作为一个独立预测指标判断大肠癌的预后。 Objective To investigate the expressions and significance of SMAD4, CyclinD1,Survivin and Cox-2 protein in colorectal carcinoma. Methods The expressions of SMAD4, CyclinD1, Survivin and Cox-2 in 44 cases of colorectal carcinoma were detected by immunohistochemical method. Results The expressions of SMAD4, CyclinD 1, Survivin and Cox-2 were no relation to sex, age, histology type and organization classification (P 〉 0.05 ). The expressions of SMAD4, Survivin and Cox-2 were significantly related to Dukes stage and lymph node metastasis of colorectal carcinoma (P 〈 0.05 ). All the factors were analyzed by COX proportional hazard model. It showed that SMAD4 was independently associated with patients' survival length. Conclusions The combine detection of SMAD4, CyclinD1,Survivin and Cox-2 may help to understand the oncogenesis and development of colorectal carcinoma. It may also help to treat tumor and evaluate prognosis. The expression of SMAD4 is an independent and accurate predictor of fine clinical outcome for individual colorectal carcinoma.
