完全腹腔镜治疗胰岛细胞瘤3例

Laparoscope in the treatment of 3 cases of insulinoma

目的 探讨腹腔镜胰岛细胞瘤切除的可行性。方法 回顾2005年3月至2006年6月我科对3例胰岛细胞瘤病人实施的腹腔镜胰岛细胞瘤切除术,手术种类包括：胰岛细胞瘤剜除术（2例）和胰体尾加脾切除术（1例）。并结合国内外有关文献报道进行诊治分析。结果 手术过程顺利,肿瘤完整切除,3例均在全腹腔镜下完成,术中出血分别为15、70、500 ml,2例术后第2天进食、下床活动,1例少量渗出。3例病人均痊愈出院。结论 腹腔镜对于胰岛细瘤切除是微创、安全可行的,具有术中出血少、创伤轻、术后恢复快、并发症少等优点,值得进一步推广。

Objective To discuss the feasibility of the laparoscopic resection of insulinoma. Methods Review the three laparoscopic resections of insulinoma for three insulinoma patients by our section office between Mar. 2005 and Jun. 2006. The three laparoscopic resections of insulinoma consist of two operations by gouging out the insulinoma and the one operation of pancreas body and afterbody and lienectomy. And analyse these operations by reading the domestic and overseas literature. Results The operations were all right, the tumors were cut off completely . The three operations were all accomplished by entirely laparoscopic. And the hemorrhages during the operations were respectively 15 m1,70 ml and 500 ml. Two patients ate and walked after the operations for the 2 days . And one patient transuded a little. The 3 patients were all healed. Concision The laparoscopic resection of insulinoma is tiny -wound, safe, feasible , and have the merits such as a little while hemorrhage , slight wound , get well quickly and few syndromes and so on . So it worth while be popularized more .
