风湿性心脏病并发心房纤颤的临床因素比较

Comparison of Clinical factor in rheumatic heart disease complicated with atrial fibrilation

目的探讨心房纤颤(房颤)与临床相关因素的关系。方法将295例风湿性二尖瓣狭窄患者根据有无持续性房颤分为AF组212例和SR组83例,术前均行常规心脏彩色多普勒检查。结果AF组患的风心病病程及年龄明显高于SR组,其左房直径、右房容积、右室容积均大于SR组,其心功能等级较SR组差,三尖瓣反流量明显高于SR组,二尖瓣口面积小于SR组以上均有显著性差异(P<0.05);中心静脉压和肺动脉嵌压均明显高于SR组;2组间的左室容积没有显著性差异。结论风心病病程、年龄、左房直径、右房容积、心功能等级、三尖瓣反流量、二尖瓣反流量、二尖瓣口面积、中心静脉压、肺动脉嵌压均可能与房颤的发生存在联系。

Objective It is to discuss the relations between atrial fibrilation and clinical factors. Methods 295 patients with rheumatic mitral stenosis were divided in two groups according to sustained AF or not. There were 212 cases in AF group and 83 cases in SR group. Conventional heart color Doppler was both checked before operation. Results RHDd was obviously longer, the age was obviously older, LAD, RAV and RVV were bigger, NYHA was inferior, TR was obviously higher, MO was smaller, CVP and PAWP were obviously higher in AF group than that in SR group. There was all significant difference on the above （P〈 0.05）. There was no significant difference on LVV between the two groups. Conclusion RHDd, age, LAD, RAV, NYHA, TR, MR, MO, CVP and PAWP may be all have relations with the occurrence of atrial fibrillation.
