肺阻抗血流图在判断风湿性心脏病患者心功能状态的价值

RHEOPNEUMOGRAPY FOR THE EVALUATION OF CARDIAC FUNCTION IN PATIENTS WITH RHEUMATIC VALVULAR DISEASE

目的：探讨风心病不同心功能时期肺阻抗血流图的变化及其临床应用价值。方法：以40例健康人作为对照组（A组），81例风心病患者按NYHA心功能分级分为B组（心功能Ⅰ级）、C组（心功能Ⅱ级）、D组（心功能Ⅲ级），用WSY-4B型四道生理记录仪在安静状态下进行。心电图、心音图、血流图（△2）及微分图（dz／dtmax）的同步描记。结果：肺阻抗血流图显示：（1）B组与A组比较差异无显著性（P＞0．05）；（2）C组与B组比较差异有显著性（P＜0．05）；（3）D组与B组之间在基础阻抗Zo值、A2－O间期、Cdz／dtmax、Hs、Hd／Ha比值间差异有显著性（P＜0．05）。结论：表明肺阻抗血流图在风心病不同心功能时期有不同表现，对了解和判断心功能状态有较好的临床价值。

Objective: To probe into the changes of rheopneu mogram (RPG) and its clinical significance in different period of the cardiac function. Methods: 40 healthy subjects as control group A and 81 patients with rheumatic heart disease(RHD)were devided into group Ⅰ B, Ⅱ C and Ⅲ D following NYHA cardiac functional system. ECG, PCG, Rheogram (△2) and dz/dtmax were described by WSY-4B Biophysics recorder int the quiet state. Results: There was no significant difference of RPG between group A and group Ⅰ B(P>0. 05) while significant difference of RPG was found between group Ⅱ C and group Ⅰ B (P<0. 05),and there were significant differences of the parameters including value of basic impedence Zo, A2-O interval,C dz/dt max, Hs and Hd/Hs between group Ⅲ D and group Ⅰ B. Conclusion: RPG vary with different period of cardiac function in patients with RHD and have certain clinical value in understanding the state of cardiac function.
