清糜导赤汤治疗顽固性乳糜血尿

本文运用补阴益气煎合导赤散,加滋阴清热、凉血止血药,组成清糜导赤汤,治疗10年以上,反复发作的顽固性乳糜血尿6例。结果取得较为满意的疗效,全部患者症状消失,蛋白血尿消失,乳糜试验转阴,除一例还在观察外,其余病例随访3～5年,未见复发。因而认为:乳糜血尿脾肾气阴两虚为其本,湿热下注是其标,对顽固性乳糜血尿治宜补阴益气为主,佐以清热利湿。

Six cases of obstinate hematochyluria with repeated attacks are reported to be treated wiih Qing Mi Dao Chi Tang (Decoction for Treating Hematochyluria), a special prescrip- tion composed of Decoction for Nourishing Yin and Replenishing Qi and Decoction for Treating Hematuria and added herbs for nourishing yin, clearing away heat, removing heat from the blood to stop bleeding. All the cases received satisfactory results: the pre- vious symptoms and signs disappeared, protein-hematuria was not found in routine urine examihation and chyle test returned negative. Only one case is still under observation, the others have not recurred after a follow-up of 3 to 5 years. It is believed that for hematochyl- uria, deficiency of spleen-qi is the primary cause while damp-heat evoling the lower-jiao is the secondary. Thus it is suggested that obstinate hematochyluria be treated mainly by nourishing yin and replenishing qi, assisted by clearing away heat and promoting diuresis.
