肺炎支原体肺炎患儿血清IL-4、IL-18及IFN-γ检测及意义 被引量:7

摘要 目的:探讨小儿肺炎支原体肺炎(Mycoplasmal pneumonia,MPP)血清IL-4、IL-18及IFN-γ的变化及其临床意义。方法:住院确诊的MPP患儿24例及正常对照组儿童21例。用ELISA测定MPP急性期、恢复期、正常对照组血清IL-4、IL-18及IFN-γ。结果:MPP患儿血清IL-4、IL-18含量急性期明显高于恢复期和正常对照组,差异有非常显著意义(P<0.01);恢复期和正常对照组比较,无统计学意义(P>0.05);重症组明显高于轻症组(P<0.05);血清IFN-γ含量急性期轻度高于恢复期和正常对照组,但无统计学意义;血清IL-4/IFN-γ比值急性期高于恢复期和正常对照组,差异有显著意义;MPP患儿急性期血清IL-4含量重症组明显高于轻症组,急性期IFN-γ含量重症组稍高于轻症组,但无统计学意义;恢复期IFN-γ、IL-4、IL-4/IFN-γ重症组和轻症组比较无统计学意义;MPP患儿血清IL-4,IL-8呈显著正相关(P<0.001)。结论:MPP患儿存在THl/TH2失衡,TH2反应占优势,IL-4及IFN-γ参与MPP患儿的免疫状态改变,在MPP发病机制中起一定作用。IL-18在肺炎支原体肺炎患儿血清中明显增高,且与IL-4呈正相关,IL-18可能在促进IL-4的分泌中起到一定的作用。
