腹腔镜直肠癌根治术的探讨

DISCUSSION ON THE LAPAROSCOPIC RADICAL OPERATION OF RECTAL CANCER

目的探讨腹腔镜在直肠癌根治术的手术方法、优点及适应症。方法自2004～2006年我院共施行了腹腔镜直肠癌根治术45例,行Dixon手术36例,Miles手术9例,其中2例中转开腹。结果手术均成功完成,手术时间180～240min,手术出血量30～200ml,无输尿管损伤,2例吻合口瘘,经保守治疗治愈,平均住院时间10.0d。结论腹腔镜直肠癌根治术具有术野广、创伤小、术后肠道功能恢复早、免疫干扰轻、术后活动早、住院时间短、手术并发症少等优点。

Objective Discuss laparoscopic resection of rectal cancer in the surgical methods, advantages and indications. Method Since the 2004 - 2006 hospital had performed laparoscopic radical surgery for rectal cancer in 45 patients, in which Dixon operation 36 cases, surgery Miles9 cases, 2 conversion. Result The operation was successfully completed, operation time 180 -240 min, surgical bleeding 30 -200 ml, no ureteral injury, two cases of anastomotic fistula cured by conservative treatment, the average length of stay is 10.0 days. Conclusion Laparoscopic resection of rectal cancer has the advantages of wide surgical field, less trauma, postoperative bowel function resume early,light immune interference, early postoperative activities, shorter hospitalization time and less postoperative complications.
