摘要
目的探讨前列腺特异抗原移行区密度(PSAT)在前列腺癌早期诊断中的作用。方法在血清PSA>4.0 ng/ml,并接受前列腺活检的144例受检者为研究对象,根椐病理学诊断分为前列腺癌组(PCa组)、良性前列腺增生组(BPH组);血清PSA含量应用瑞典CanAG公司试剂盒;经直肠超声测量前列腺癌体积和移行区体积,分别计算前列腺特异抗原密度(PASD)和PSAT。应用SPSS(10.0)软件进行统计学分析。结果(1)受检者的前列腺移行区体积与前列腺总体积呈明显正相关(r=0.78,P<0.001)。(2)BPH组与PCa组比校,总体积无显著差别(P>0.05),前列腺移行区体积、PSA、PSADP、SAT均有显著差别。ROC曲线显示PSAT的曲线下面积(AUC)为0.771大于PSA、PSAD,且有显著差别(P<0.01)。(3)在灰色区域,PSAT在BPH组和PCa组中存在显著差别;ROC曲线表明PSAT强于PSAD。结论前列腺特异抗原移行区密度是提高前列腺癌早期诊断效率的有效指标。
Objective To stduy the value of prostate specific antigen of transition zone (PSAT), on early diagnosis of prostate can- cer.Methods During the prostate cancer screening, 144patients were selected into the research with the pathological stages;and were divided PCa group and BPH group.The serum PSA was measured with CanAg PSA kit;and the patients'prostate total volume,transition zone volume (TZ) were recorded,PSAT were calculated with the formula: PSAT = PSA/TZ.All the data manage with SPSS10.Results The result showed that ( 1)Between BPH and PCa group,prostate total volume ( r = 0.78, P 〈 0.001). (2)Between BPHand PCa group, prostate total volume has no significance ( P 〉 0.05), TZ, PSA, PSAT, PSAD, all has significance ( P 〈 0.05 ). ROC Curves revealed PSAT has better sensitivity and specificity on detecting prostate cancer (AUC for PSAT:0.771). (3)In the grey zone,PSAT was more significance than PSAD between BPH group and PCa group. Conclusion PSAT is better diagnosis marker in early diagnosis in prostate
