鼻道窦口复合体区的解剖观察及临床意义

The Anatomical Observation and Clinical Significance of the Ostiomeatal Complex

目的:通过对50例骨性鼻腔外侧壁的观测,旨在了解鼻道窦口复合体区的解剖结构及变异情况,为鼻内窥镜手术提供解剖学依据。方法:观察50例骨性鼻腔外侧壁标本的窦口鼻道复合体区的解剖结构及变异情况,测量有关数据并将结果进行统计分析。结果:上颌窦自然孔与中鼻甲前缘附着处的距离为15±0.85 mm,与鼻泪管的距离为9±0.25mm。变异包括中鼻甲反向弯曲(14%)、气化(20%),钩突肥大(8%)、偏曲(10%)、气化(2%),筛泡过度气化(8%),鼻丘过度气化(2%)。结论:鼻道窦口复合体区域解剖结构及毗邻关系复杂,解剖变异常见,故手术前详细检查,对设计合理的手术方案,安全地施行手术有重要的临床意义。

Objective ：To provide anatomical basis for Functional Endoscopic Sinus Surgery （FESS） through anatomic study of the Ostiomeatal Complex（OMC）. Methods： To observe the normal anatomy and the variations of the OMC of osseous mycteric lateral walls on 50 cases. Results. The distance between the antral ostia and the prozone of middle turbinate is 15：k 0. 85mm, The distance between the antral ostia and the nasolacrimal duct is 9 ± 0. 25mm. Anatomic variations include paradoxical middle turbinate （14%）, gasify （20%） ,uncinate process hypertrophy（8%）, deviation（10M） ,gasify（2%）, ethmoid bulla hypergasify（8%）, agger nasihypergasify（2%）. Conclusion： The construti-on and syntopy of the OMC are very complicated, anatomic variations of the OMC are common findings. The all-round examination before FESS is very important for safe operation.
