复方樟柳碱治疗前部缺血性视神经病变的疗效观察

Therapeutic Effect of Compound Anisodine on Anterior Ischemic Optic Neuropathy

目的观察复方樟柳碱治疗前部缺血性视神经病变(anterior ischemic optic neuropathy,AION)的疗效。方法AION48例50只眼,25例26只眼为观察组,接受复方樟柳碱治疗;23例24只眼为对照组,接受溶栓抗凝及血管扩张剂治疗。14d为一个疗程,共2个疗程。观察用药前后视力、眼底血管及视野的变化,综合判断疗效。结果观察组平均视力0.75±0.23,对照组0.47±0.24;观察组中24只眼(92.3%)视野好转,2只眼(7.7%)无明显改善,对照组中12只眼(50%)视野好转,10只眼(41.6%)无明显改善,2只眼恶化,无法检查视野,观察组视力及视野较对照组明显好转(<0.05)。观察组24眼视盘充盈明显改善(92.3%),2眼(7.7%)还存在充盈不良的状况,明显优于对照组10眼视盘充盈明显改善(41.7%),14眼(58.3%)还存在充盈不良的状况(<0.01)。结论复方樟柳碱治疗AION疗效好,无副作用,优于传统治疗方法。

Objective To observe the therapeutic effect of compound anisodine on anterior ischemic optic neuropathy （AION）. Methods In 48 patients （50 eyes） who were diagnosed as with AION, 25 patients （26 eyes） underwent superficial temporal intravenous drip with compound anisodine （2ml/d） （treatment group）, and the other 23 patients （24 eyes） underwent intravenous drip with the kind ofthrombolytic coagulant or vasodilator （once per day） （control group） during the treatment period of 28 days. The changes of visual acuity and fundus fluorescence angiography（FFA） of the patients after injection were recorded, and the visual field was counterchecked in the patients who had undergone the examination before the treatment. Results The visual acuity and visual field was better in the treatment group than that in the control group The visual acuity in the treatment group and the control group was 0.75±0.23 and 0.47±0.244, respectively. In 24 eyes in the control group which had undergone examination of visual field, the visual field improved in 12（50%）and of remained still in 10 （41.6%）. In 26 eyes in treatment group which had undergone examination of visual field, the visual field improved in 24 （92.3%） and remained still in 2 （7.7%）. The number of eyes which vessels of optic disc were dilated significantly in the treatment group is superior to that of the control group（ P 〈 0.0 1）. Conclusion Compound anisodine is efficacious in treating AION.
