睾丸畸胎瘤的彩色多普勒超声表现及其诊断价值

Color Doppler ultrasonographic findings of testicular teratoma and their diagnostic value

目的探讨睾丸畸胎瘤的彩色多普勒超声表现及其诊断价值。方法回顾性分析经病理证实的11例睾丸畸胎瘤的二维声像图,彩色多普勒及脉冲多普勒频谱特征。结果在11例睾丸畸胎瘤中,恶性畸胎瘤2例(18%),良性畸胎瘤9例(82%)。本组睾丸畸胎瘤超声表现共同特征是:患侧睾丸增大,内部可见实质性或混合性回声团。彩色多普勒超声显示睾丸恶性畸胎瘤内部及周边血流信号丰富,杂乱,动脉血流频谱(PSV:6～7cm/s,RI:0.34～0.37),睾丸良性畸胎瘤内部及周边无或偶见点状血流信号。结论彩色多普勒超声诊断睾丸畸胎瘤具有较高的准确性,是诊断睾丸畸胎瘤的主要方法。

Objective To evaluate the value of color Doppler ultrasonography（CDUS）in the diagnosis of testicular teratoma. Methods The 2D ultrasonographic, CDUS and pulsed wave Doppler ultrasonic features of 11 patients with pathologically proved testicular teratoma were analyzed retrospectively. Results Among 11 patients, malignant teratomas were found in 2 cases（ 18% ）, benign teratomas were found in 9 cases （82%）. The common ultrasonic features of testicular teratomas in the group were enlargement of affected testis and intratesticular solid or mixed echo masses. CDUS showed that, malignant testicular teratomas whose inside and periphery had rich blood flow signals and arterial flow frequency -spectrum （PSV 6 - 7 cm/s, RI 0.34 -0. 37）, while benign testicular teratomos whose inside and periphery lacked or occasionally had punctual flow signals. Conclusion CDUS has high accuracy in the diagnosis of testicular teratoma so that it should be taken as a main method to diagnose this disease.
