缓释粒子内镜植入在胃肠道肿瘤化疗中应用观察 被引量:5

摘要 目的:初步评价化疗缓释粒子肿瘤部位植入治疗胃肠恶性肿瘤的临床疗效和不良反应。方法:选择隆起型胃肠道恶性肿瘤患者31例,在消化内镜直视下将5-FU、顺铂的缓释粒子植入到肿瘤间质。结果:完全缓释10例,部分缓释12例,病情稳定6例,3例无缓解,平均有效率为70.9%。患者疼痛缓解率为70.9%,体重增加21例,占67.7%。不良反应除因术中操作出血外,仅见WBC轻度下降、轻度恶性、腹泻,无发生肝肾功能损害。结论:胃肠道恶性肿瘤化疗缓释粒子内镜植入突出了局部靶向治疗,具有微创、安全、有效及并发症发生率低的特点,是综合治疗胃肠道恶性肿瘤新的有效手段之一。 Objective:To evaluate the therapeutic efficacy and side effect of endoscopic implantation with chemotherapeutic slow-release ion for the treatment of gastrointestinal malignant tumors.Methods:Patients with gastrointestinal malignant tumors typed as protruded lesion were chosen. Then slow-release ion of 5-FU and cisplatin were implanted in the patients'tumor matrix through endoscopy.Results:All the operations were successfully performed.In all cases, 10 had complete relief, 12 partial relief, 6 stable and 3 non-response, with the average response rate of 70.9%.Pain relief and body weight gain rate were 70.9%and 67.6%,respectively.Except inevitable trifling hemorrhage during the course of endoscopy,the main side effects were slight nausea, diarrhea and decrease of WBC,without any liver or renal damage.Conclusions : The treatment of gastrointestinal malignant tumors with chemotherapeutic slow-release ion endoscopic implantation emphasizes local targeting treatment and shows the characteristics of security, efficiency, minimal injury and low rate of complications.It is one of the effective therapies for gastrointestinal malignant tumors.
