摘要
目的探讨穿心莲对实验性肝硬化大鼠肠道细菌移位的作用,为其临床应用提供依据。方法将30只肝硬化模型大鼠随机分为4组:穿心莲(AN)组[n=8,穿心莲0.5g/(kg.d)],诺氟沙星(NF)组[(n=8,诺氟沙星0.3g/(kg.d)],穿心莲+诺氟沙星(AN+NF)组[n=8,穿心莲0.5g/(kg.d)+诺氟沙星0.3g/(kg.d)],肝硬化对照(CN)组[n=6,生理盐水2ml/(kg.d)],治疗2周后结束。比较各组大鼠死亡率,取外周静脉血检测血浆肿瘤坏死因子α(TNF-α)、一氧化氮(NO)水平;取肠系膜淋巴结、血液、肝、脾和回肠内容物做细菌培养;采用高压液相色谱法检测肠黏膜通透性;部分肝脏和回肠组织作病理切片检查。结果与CN组相比,AN、NF和AN+NF组的TNF-α、NO浓度显著降低(均P<0.05),回肠细菌计数和细菌移位发生率显著下降(均P<0.05),AN和NF组的肠道通透性显著下降(均P<0.05)。病理检查发现CN组肝硬化大鼠回肠绒毛变短、破坏,炎症细胞浸润增多,而其他3组的肠绒毛结构改变不明显。结论穿心莲能有效阻断肝硬化大鼠的肠道细菌移位。
Objective To evaluate the effects of andrographitis on bacterial translocation in experimental cirrhotic rats.Methods Thirty rats with CCl4-induced cirrhosis were randomly divided into 4 groups:group AN [n=8,andrographitis 0.5 g/(kg·d)],group NF [n=8,norfloxin 0.3 g/(kg·d)],group AN+NF [n=8,androgtaphitis 0.5 g/(kg·d)+norfloxacin 0.3 g/(kg·d)],group CN [n=6,0.9% normal saline 2 ml/(kg·d)].After treatment for 2 weeks,the mortality of each group was analyzed.Blood specimens were collected from peripheral vein for detections of serum levels of ALT,AST,TNF-α and NO.The MLNs,blood,liver,spleen and contents of ileum were obtained and the samples were cultured.Intestinal permeability was measured by HPLC with RID.Parts of liver and ileum were cut into sections for pathological examinations.Results Compared with group CN,the serum levels of TNF-α and NO in group AN,group NF and group AN+NF were decreased significantly(P〈0.05) and the intestinal permeability in group AN and group NF was also decreased remarkably(P〈0.05).Total ileum bacterial counts and incidence of bacterial translocation in group CN were significantly higher than in other groups(P〈0.05).The ileum histological appearance in group CN was obviously abnormal,and villi were shortened and damaged with submucosal inflammatory infiltration.Conclusion Andrographitis can effectively prevent cirrhotic rats from developing bacterial translocation in intestinal tract.
