摘要 目的开发基于CT图像的后装治疗模拟计划系统,并用JR1152-F型热释光剂量计(TLD,1mm×1mm×6mm)和仿真人体盆腔模型(CDP)验证该系统的准确性。方法用VC++开发此后装治疗模拟计划系统(3DV-TPS),仿真人体盆腔模型在GE LightSpeed^(16) CT上扫描后,其图像经网络传输至3DV-TPS上进行腔内后装治疗模拟计划设计,计算出各CT层面上等剂量线分布和各测试点的剂量值,再把TLD元件放入仿真人体盆腔模型测试孔内,按模拟计划进行腔内后装照射,采用FJ427-A1热释光剂量仪对受照后TLD元件进行测量,并比较计算值和测量值的差异。结果此3DV-TPS实现了基于CT图像的后装治疗模拟计划设计,等剂量曲线可在任意横断面、矢状面、冠状面上精确显示,剂量分布与组织器官的相互关系直观形象。实测剂量值与3DV-TPS计算值符合较好,相对偏差在±7.7%以内。结论3DV-TPS准确可靠,用JR1152-F型TLD和仿真人体盆腔模型验证宫颈癌腔内后装治疗三维剂量分布具有可行性。 Objective To develop a set of HDR ^192Ir 3D visual after-loading brachytherapy simulating planning system (3 DV-TPS) based on CT images and investigate the accuracy of the 3 DV-TPS by JR1152-F TLD ( 1 mm × 1 mm ×6mm ) and anthropomorphic pelvic phantom ( CDP). Methods The 3DV-TPS based on CT images was developed with Visual C + + program language. The phantom was scanned by GE LightSpeed^16 CT machine and the CT images were transferred to the 3DV-TPS to create a simulating brachytherapy plan. The isodose distributions and the doses of all measured positions were calculated according to the prescription doses of reference points, then TLDs were put into the measured holes of the CDP. After the simulating treatment plan was delivered, the doses to each TLD were measured by a FJ427-Al thermoluminescence dosimeter and the measured values were compared with the calculated values by the 3DV-TPS. Results The 3D visual brachytherapy plan based on CT images can be created by the 3DV-TPS. The isodose distributions can be accurately displayed on any axial,coronal or sagittal CT images. The calculated and the measured dose values are in good coincidence in all of the reference points. The relative errors for those positions are within ± 7.7%. Conclusion The 3DV-TPS is accurate and reliable. The JR1152-F TLD and CDP are credible in checking the 3D-dose distributions of ^192 Ir HDR intracavitary after-loading brachytherapy for patients with cervical carcinoma.
