摘要
目的:探讨利用经鼻十二指肠营养管行肠内营养在食管重建术中的应用和价值。方法:160例胸上段食管癌患者随机分为肠内营养组和静脉营养组,肠内营养组均于术中插入十二指肠营养管达十二指肠远端30~40cm,术后第2天开始滴注营养液行肠内营养;对照组按常规方法行术后静脉营养支持。两组患者分别于术后第10天检测体重、血浆白蛋白、前白蛋白,观察术后肛门排气时间,并于病人出院后计算总住院费用。结果:两组患者的体重、白蛋白无明显差异,前白蛋白则存在明显差异;肠内营养组术后肠道功能恢复时间早于对照组,住院总费用显著低于静脉营养组。结论:对于食管重建术病人,用鼻十二指肠营养管行肠内营养能较静脉营养更好的改善患者营养状况,促进术后肠道功能的恢复,减少住院费用。
Objective To evaluate the effect of naso-duodenum nutrition tube on nutrition support after operation of esophagus carcinoma.Methods 160 patients were randomly divided into two groups:enteral nutrition group and intravenous nutrition group.The patients in enteral nutrition group accepted naso-dudenal nutritiontube during operation and nutrition liquid from nutrition tube in the postoperative day.The patients in intravenous nutri- tion group accepted routine nutrition liquid by vein.The body-weight,levels of plasma albumin and prealbumin were detected on the 10th postoperative day respectively,and the total cost of hospitalization was calculated.Results There was no significant difference between two groups in body-weight and level of plasma albumin.The level of plasma prealbumin was higher in enteral nutrition group than in intravenous nutrition group(A).The time of gastrointestinal function recovery in enteral nutrition group was shorter than in intravenous nutrition group(*). The total cost of hospitalization was lower than intravenous nutrition group.Conclusion Nutrition support through naso-dudenal nutrition tube for patients undergoing upper thoracic esophagectomy may improve nutritional state better,promote the reablement of intestinal function,and decrease the total cost of hospitalization.
