摘要
目的:探讨食管癌术后发生心律失常的危险因素以指导临床防治。方法:收集2006年1月—2006年12月887例食管癌手术患者资料,对术后心律失常可能的危险因素进行分析。结果:年龄超过60岁、术前心电图异常、高血压、FEV1/FVC<70%、手术时间>3.5h、术中失血>600ml、弓上及颈部吻合等是食管癌术后发生心律失常的危险因素。结论:对具备以上因素的患者应在术前有效控制血压,加强心肺功能锻炼,改善营养状况,术中轻柔操作,术后给予有效镇痛、鼓励有效排痰、保持胃管引流通畅。
Objective:To Study the risk factors of cardiac arrhythmia in postoperative patients with esophageal carcinoma for preven tion. Methods: The clinical data of 887 patients with cardiac arrhthinia after esophageal cancer surgery in our hospital from January 2006 to December 2006 were collected and analyzed retrospectively. Results: Age older than 60y, abnormal cardiogram before operation, hypertension, FEV1/FVC〈70% ,operation time beyond 3.5 h, blood loss during operation 〉600 ml, neck and supra-aortic arch anastomosis were risk factors in the development of arrhythmia after esophageal carcinoma operation. Conclusion: Blood pressure con- trol, heart and lung function exercise,gently operation, pain-relief, sputum removal and stomach tube drainage can prevent arrhythmia or decrease the incidence.
