功能性消化不良患者胃窦部容积和胃排空时间与临床症状关系的超声评估

Relationship among gastrointestinal symptoms,gastric emptying and artral volume in functional dyspeptic patients with ultrasonograghy

目的评价功能性消化不良(FD)患者上消化道症状、胃排空时间及胃窦部容积(AV)之间的关系。方法选取40例FD患者,根据临床症状进行评分分组;另选取20例健康者为对照组。超声测量40例FD患者及20例健康者的胃排空时间及AV。结果FD运动障碍亚型中67.5%的患者有胃排空异常;FD患者禁食时及实验末的AV均较对照组扩张(P<0.05);胃排空延迟的FD患者中,禁食时和实验末期AV分别为(17±10)ml和(46±11)ml,明显大于对照组及胃排空正常的FD组相应的AV(P<0.05),而胃排空正常的FD患者的禁食时及实验末期的AV与对照组比较无统计学意义(P>0.05)。FD患者禁食时AV超过15ml预测胃排空延迟的敏感性、特异性分别为41.9%、26.2%。结论胃排空延迟与运动障碍样症状有关,AV扩张与胃排空延迟有密切相关性,禁食时FD患者的AV超过15ml时可以预测胃排空延迟的存在。

Objective To access the relationship among gastrointestinal symptoms, gastric emptying and artral volume in functional dyspeptic patients.Methods Fourty patients with FD were scored and qrouped respectinely according to their cilinic symptom, and 20 health subjects as contral group the antral volume and gastric emptying were assessed with ultrasonograghy. Results Delayed gastric emptying was detected in 67.5 % of the dysmotility - like symptoms in patients with FD. In FD patients, the mean fasting antral volume and the final antral volume was larger than that those in healthy subjects（ P 〈 0.05） ,the mean fasting AV （ 17 ± 10） ml and the final AV（46± 11） ml was larger in patients with delayed gastric emptying than those with normal gastric emptying and in healthy subjects, but the mean fasting AV and the final AV in patients with normal gastric emptying was not significant（vs controls）. This study showed that when the fasting antral volume 〉 15 ml, the sensitivity and specificity predicting the delayed gastric emptying is 41.9% ,26. 2% respectinely. Conclusion The dysmotility - like symptoms in patients with FD may be attributed to gastric emptying. Enlarged antral volume was significantly associated with delayed gastric emptying. This study shows that the fasting antral volume of 15 ml is the cut - off value that may be used to predict delayed gastric emptying.
