摘要
目的:探讨磁共振扩散张量成像(DTI)及扩散张量纤维束成像(DTT)在脑膜瘤中的应用价值。方法:27例经手术病理证实的良性脑膜瘤患者,术前全部行头部MRI平扫及增强扫描,并加做DTI序列。结果:27例脑膜瘤患者,13例肿瘤累及锥体束,其中11例肌力有不同程度下降;4例累及视放射导致视力障碍;其余肿瘤未涉及重要神经传导束。结论:DTI、DTT能直观的显示肿瘤与重要神经纤维束的关系,与临床症状有良好相关性。对指导涉及重要神经纤维束的脑肿瘤手术及判断预后具有重要临床价值。
Objective:To explore the clinical role of diffusion tensor imaging (DTI) and diffusion tensor tractography (DTT) in meningiomas. Methods:Twenty-seven patients with benign meningiomas underwent conventional MRI (before and after administrate the contrast)and DTI before surgery. Results:Of the 27 patients, the tumors involved pyramidal tracts in 13,11 of which accompanied with descendent muscle power ;the tumors involved optic radiation and leading to visual disorder in 4; The others did not involve vital nerve conduction bundle. Conclusion:DTI,DTT can show the relationship between the tumors and the vital nerve fiber bundle directly, and have a good correlation with the clinical symptoms. It is found to be of great value in the surgical planning in tumors which invole the vital nerve fiber bundle or prognosticating the prognosis of patient.
