螺旋型鼻肠管不同冲管方式的比较

目的选择一种更有利于患者的螺旋型鼻肠管（简称鼻肠管）冲管方法,以确保患者顺利进行鼻肠管营养。方法2005年8月～2007年6月对60例留置鼻肠管予营养液进行肠内营养（1周以上）的患者随机分为对照组与实验组各30例,对照组采用温开水按照卜献磊等提供的方法进行冲管,冲管时、冲管后将管末端提高,将管腔内的鼻饲液排空,每8 h冲洗管道1次和在鼻饲营养液完毕后冲洗管道1次,每次30 ml;实验组只在鼻饲营养液完毕后,先用10 ml注射器予10 ml温开水以脉冲式注入后再向管腔内注入20 ml并夹管,将冲洗液保留于鼻肠管腔内。比较两组堵管率、冲管例次数、冲管所导致的非治疗性摄入量情况。结果两组均无发生堵管情况,冲管次数实验组仅是对照组的1/3,冲管所致的非治疗性摄入量实验组仅是对照组的1/9。结论实验组采用的冲管手法能减轻护理人员的工作量,减少非治疗性摄入量,对配合医生制定患者的治疗方案有积极性意义,更适用于鼻肠管冲管。

Objective To find out a tube-washing good for patients to ensure smooth performance of nutrition by spinal nasointestinal tube. Methods 60 patients with enteral nutrition via spinal nasointestinal tube, admitted in the hospital during Aug. 2005 to June 2007, were randomized equally into experimental group and control group. In the control group, the tubes were washed via the method by Piao Xian-lei. At washing or after washing, the tube end was raised up to empty the nasointestinal fluids. The tubes were cleaned once every 8 hours and washed once after finish of nasointestinal fluid. In the experimental group, 10 ml boiled water were injected with a 10 ml syringe and 20 ml of boiled water were injected, then the tube was clipped. The cleaning solution was kept in nasointestinal tube. The tube-blocking rate, times for washing tubes, non-treating intake were compared. Results No blocking of tubes happened in both groups. In tube-washing, the times in the experimental group was only one-third times of the control group. The non-treating intake in the experimental group was only one-ninth of the control group. Conclusions The washing method in the experimental group can reduce the tense and workload for the nurses. Reduction of non-treating intake is important for their cooperating with doctor and suitable for washing nasointerstinal tubes.
