腓肠肌内侧头肌皮瓣修复小腿周围皮肤缺损

Reconstruction the soft tissue defect around leg by the medial head of gastrocnemius musculocutaneous flap

目的探讨小腿周围皮肤缺损的修复方法。方法利用腓肠肌内侧头肌皮瓣移位进行修复小腿周围皮肤软组织缺损临床应用19例。结果经手术和药物治疗,1例远端皮肤部分坏死;2例皮下血肿,经引流后消失,余肌皮瓣存活良好,胫骨骨折处骨痂生长良好,骨髓炎得以控制。平均随访39.3月,踝关节活动正常;除1例膝部功能轻度受限外(原发损伤广泛合并复杂骨折),余功能基本正常。结论腓肠肌内侧头肌皮瓣治疗此类损伤符合生理要求,且重建修复手术一次完成。

Objective To investigate the methods of repairing the soft tissue defect around leg. Methods A new method of extending the use of the medial head of gastroenemius musculoeutaneous flap was performed,and applied to 19 patients clinically. Results The average follow - up was 29.3 months. All the wounds had the primary healing. The activity of ankle joint was normal in the whole group. There were two patients whose knee function was limited slightly （the initial injury was extensive, with complex fractures of the knee）, the other patients＇ hip functions was basically normal. Conclusions The simple and practical method fits the physiology reconstruction and the repair operation was completed once.
