摘要
目的总结肝硬化患者行腹腔镜胆囊切除术的临床经验。方法对接受腹腔镜胆囊切除术的73例合并肝硬化患者的治疗结果进行回顾性分析。结果腹腔镜胆囊切除术手术成功68例,中转开腹5例,中转开腹原因:三角区致密粘连2例,术中出血、镜下止血困难3例。术后肺部感染3例,无胆道损伤和肝功能衰竭等严重并发症,患者均治愈出院。结论严格掌握手术指征和特点,并加强围手术期的处理,肝硬化患者行腹腔镜胆囊切除术是安全可行的。
Objective To summarize clinical experiences of laparoscopic cholecystectomy（LC） combined with cirrhosis. Methods Combined with liver cirrhosis,the clinical records of 68 cases treated by LC were retrospectively analyzed. Results LC was successfully performed in 68 cases and a conversion to open surgery was required in 5 cases because of dense adhesion of Calot triangle in 2 cases and difficult management of bleeding under laparoscopy in 3 cases. There were 3 cases of respiratory tract infection, but no severe complications occurred. Conclusions LC can be performed safely in the majority of cases of cirrhotic patients. It is important to know the characteristics and to pay attention to the peri -operation management.
