非小细胞肺癌MSCT征象与CD44v6、FHIT及p16表达间的临床意义

摘要 目的探讨非小细胞肺癌的MSCT征象与其生物行为,CD44v6、FHIT及p16表达间的关系。方法43例非小细胞肺癌病人术前一周内行MSCT平扫及增强扫描。所有的标本均经10%福尔马林液固定,石蜡包埋,采用SP(streptavidin peroxidase)法检测CD44v6,FHIT及p16的表达。实验数据经SPSS11.0统计软件包进行统计学处理。结果1.MSCT对诊断临近胸膜受侵及淋巴结转移与手术病理有较好的一致性。2.CD44v6在肺癌和癌旁组织中的表达差异有统计学意义(P<0.05)。在MSCT上表现为淋巴结转移和胸膜侵犯组与其对照组有显著性差异(P<0.05)。3.p16和FHIT在癌旁组织中的阳性表达明显高于肺癌组织,其差异具有统计学意义(P<0.05)。p16阳性表达率在MSCT上表现为淋巴结转移阳性组和周围胸膜侵犯阳性组,与相应阴性组相比差异具有统计学意义。FHIT在MSCT胸膜侵犯组与相应阴性组相比较,差异具有统计学意义(P<0.05)。结论1.CD44v6的异常高表达与NSCLC的临近结构侵犯和淋巴结转移具有相关性,提示其在肺癌的浸润和转移机制中起重要的作用。2.p16与FHIT在NSCLC中常表达缺失,作为两个抑癌基因,其下调表达参与了NSCLC的发生和发展过程。3.结合非小细胞肺癌的MSCT的表现及CD44v6、p16及FHIT的异常表达对选择理想的治疗方案具有重要的意义。 Objective To investigate the correlation between Multi -slice CT features and the expression of CD44vt,FHIT and p16 in non small cell lung cancer( NSCLC ). Methods Forty - three cases with non - small cell pulmonary carcinoma were subjected to plain - scan and contrast - scan of chest within a week before surgery with GE Light Speed 16 CT scanner. All patients underwent contrast scan. All the postoperative specimens were fixed in 10% neutral formalin and embedded in paraffin. Streptavidin peroxidase (SP) immunohistochemical method was applied to detect the expression level of CD44vt, FHIT and p16. Statistical analysis was performed with SPSS 11.0 software, Resuits 1. The results of diagnosis for adjacent pleura invasion and lymph nodes metastasis by MSCT were consistent with findings of surgical pathology. 2. The expression of CD44v6 in the tumors was significantly higher than that in the paracancer( P 〈 0.05 ). The expression of CD44v6 in the group of lymph node metastasis and in the group of pleural invasion on MSCT was statistically different from their control groups respectively. 3. The expression of FHIT and p16 in lung cancer was significantly lower than that in paracancer( P 〈 0.05 ). The expression of p16 in the group with lymph node metastasis and in the group of pleural invasion on MSCT was statistically different from their control groups respectively. Conclusions 1. The over - expression of CD44v6 was closely associated with the lymph node metastasis and adjacent pleura invasion, which indicated that CD44v6 may play an important role in invasion and metastasis. 2. The deletion expression of FHIT and pl6 gene expression frequently occurred in NSCLC. And the abnormal expression FHIT and p16 may act on oncogenesis and progression of human NSCLC, and they are the negative regulatory genes and tumor inhibitors. 3. MSCT diagnosis combined with detection of the expression level of CD44v6, FHIT and p16 could favor optimal therapies for patients with NSCLC.
