Removal of carve bone of midline wedge to correct broad chin deformities

目的:探讨颏部锲形骨段切除治疗方颏的可行性。方法:全身麻醉。口内进路,在左右下牙4到4之间切开唇龈沟粘膜,肌层,骨膜。在中线用小裂钻标记好中线位置,在左右下牙6到6之间截骨,截骨完成后,再于颏部中央截取底边在舌侧的等腰三角形骨块,将颏部骨块内旋,在唇侧表面以微型钛板固定。再将颏部骨块与下颌骨体前方固定。结果:32例就医者伤口均一期愈合,外形均比较满意,长期随访,就医者出院后呼吸睡眠平稳,无一例就医者出现鼾症加重症状。结论:颏部锲形骨段切除是治疗方颏畸形的有效手段。

Objective To study the feasibility of removal of carve bone of midline wedge to correct broad chin deformities.Methods Incision was selected at bottom of vestibular groove to double canine teeth zone.After routine exposure of the symphysis through a full-thickness mucoperiosteal incision in the mandibularanterior vestibule,through intraoral approach,under general anesthesia.Drills in the median line with the small crack marks the good median line position,The horizontal osteotomy is then completed with a saw,and removal of midline wedge of bone from a lingual approach.rotation of the mobile segment to narrow chin.The plate can now be bent medially to achieve the desired transverse dimension of the chin,and the segment stabilized with rigid fixation lateral to the midline plate with screws or plates.Results 32 patients gained satisfied contour with wound homogeneous time heals,the patients breathe calmly without obstructive sleep apnea hypopnea syndrome with the long-term revisit.Conclusion Removal of carve bone of midline wedge is an effective method to correct broad chin deformities.
