摘要
目的探讨大鼠急性肺损伤(acute lung injury,ALI)大鼠模型早期肺纤维组织增生情况及其可能的机制。方法应用尾静脉注射油酸(oleic acid,OA)复制大鼠ALI模型。36只健康SD大鼠随机分为3组:OA致伤1天组(A组,n=12)、OA致伤2天组(B组,n=12)、生理盐水对照组(C组,n=12)。观察肺组织病理变化及湿/干重比(W/D)值,进行动脉血气分析,支气管肺泡盥洗液(BALF)总蛋白(TP)、总磷脂(TPL)、磷脂酰胆碱(PC)和Ⅲ型前胶原N端肽(PⅢNP)浓度测定。结果与C组比较,A、B组肺组织见明显充血、水肿、出血等表现,BALF中TPL、PC/TPL明显减少(P<0.01),而TP、PⅢNP浓度及W/D值显著升高(P<0.01),动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)、pH值也有明显变化(P<0.01);与A组比较,B组肺组织改变进一步加重,BALF中PⅢNP浓度进一步升高(P<0.01)。结论ALI早期肺脏可发生纤维组织增生,且随时间延长而加重。
Objective To investigate pulmonary fibroproliferation occured early in the acute lung injury (ALI) rats and the mechanism of pulmonary fibroproliferation. Methods The model of ALI in rats was reproduced by injecting oleic acid into tail vein. Thirty six adult Sprague-Dawley rats were randomly divided into three groups: ALI group induced by injecting oleic acid for 1 day (A group, n=12), ALI group induced by injecting oleic acid for 2 day (B group, n=12), normal controled group treated by injecting saline (C group, n= 12) . The efficacy of this experiment was evaluated by the chang of histopathology, wet/dry weight ratio (W/D) , arterial blood gas analysis, and the total proteins (TP), total phospholipids (TPL), phosphatidylcholine (PC), aminoterminal peptide of type Ⅲ proeollagen (PⅢNP) concentration in the bronchoalveolar lavage fluid (BALF) . Results Compared with the C group, there were more obvious pathological manifestations of ALI, such as hyperemia, edema , hemorrhage in the A and B group. TPL and PC/TPL in the BALF were decreased significantly (P〈0. 01), whereas TP, PⅢNP concentration and W/D were increased significantly (P〈0.01) . There was significant difference in PaO2, PaCO2, pH (P〈0.01) . Compared with the A group rats, the B group rats' changes of lung pathology were severer and the PⅢNP concentration were increased significantly (P〈0. 01) . Conclusion Pulmonary fibroproliferation occur early in ALI, and deteriorat progressively.
