血管内超声在左冠状动脉主干病变诊断中的选择性应用

摘要 目的探讨血管内超声(IVUS)在左冠状动脉主干(左主干)病变诊断中的选择性应用。方法选择性冠状动脉造影(CAG)显示左主干正常或近似正常,同时接受了IVUS检查左前降支(LAD)和(或)左回旋支(LCX)的患者,共242例作为研究对象,分析其中有或伴有左主干严重病变患者的临床及CAG特征。左主干严重病变定义为最小管腔面积(MLA)≤6.0cm^2,非严重病变定义为MLA>6.0cm^2。结果IVUS发现左主干严重病变45例,非严重病变197例。左主干严重瘸变组更多见于合并前降支近段病变及主干钙化(P<0.05或0.01)患者。结论在CAG显示左主干近似正常的患者中,前降支近段病变及左主干钙化是选择IVUS检查的重要指标。
